I (parent / guardian) , in consideration of Young Star Soccer Academy ,provide my consent and agreement for the use of photographs and/or videos taken of myself/my child during their participation in activities organised by the Young Star Soccer Academy. I understand that these photographs and/or videos may be used for promotional and educational purposes. I hereby grant the Young Star Soccer Academy, its employees, agents, and authorised representatives the irrevocable right and permission to use, reproduce, publish, distribute, and display any photographs and/or videos of my child taken in connection with their participation in the academy's activities. This includes, but is not limited to, use in brochures, newsletters, websites, social media platforms, advertisements, and other promotional materials. I understand that my child's full name will not be published or associated with any images without obtaining additional consent. The Young Star Soccer Academy will take reasonable measures to protect the privacy and safety of my child's name in accordance with applicable laws and regulations. I acknowledge that the photographs and/or videos will become the property of the Young Star Soccer Academy, and I hereby waive any right to inspect or approve the final materials that may be used. I release and discharge the Young Star Soccer Academy, its employees, agents, and authorised representatives from any claims, damages, or liability arising out of or in connection with the use of the photographs and/or videos. This consent and waiver shall remain valid unless and until revoked by me in writing. I understand that revoking consent may not affect any photographs or videos already in use by the Young Star Soccer Academy. By signing below, I acknowledge that I have read and fully understand the contents of this photo waiver consent, and I voluntarily agree to its terms and conditions.
